Rim a rocks or cocktail glass with lime and dip in kosher salt; fill the glass with fresh ice.
Fill a cocktail shaker halfway with ice. Add tequila, triple sec, pineapple juice, lime juice, and agave syrup (if using).
Shake for about 15 seconds until the shaker feels very cold.
Strain into the prepared glass over fresh ice.
Slowly add the heavy cream (or coconut cream), stirring gently with a bar spoon to swirl and combine.
Garnish with a pineapple wedge or lime wheel and serve.
Notes
Use Cointreau for a cleaner orange note. Fresh pineapple juice gives the brightest flavor; canned works in a pinch. For a lighter version, skip the cream and add a splash of soda water. Keep ingredients cold to maintain a crisp balance.
